DSM-5-TR diagnosis of BPD requires clinician judgment, longitudinal observation, and ruling out differential diagnoses including mood disorders, PTSD, substance use, and other personality pathology. Criterion B specifies that the pattern must be stable, of long duration, and traceable to adolescence or early adulthood.
